Overview
The Vital Statistics Branch registers deaths and provides deaths certificates of deaths that have occurred in the Province of New Brunswick.
Eligibility
Any person with a relationship with the person named on the certificate and who provides a reason, the correct information and fee may apply.
Description
The information required to obtain a death certificate: name of deceased, date of death, place of death, spouse's name, date of birth of deceased, name of Funeral Home(if known), relationship to person named on certificate, reason why certificate is required, daytime phone number and return address.
The death certificate includes:
name of the deceased
date of death
place of death
date of birth
province or country of birth of the deceased
registration number
registration date, and
date issued
Method of Payment: Cheque or money order payable to Service New Brunswick, Visa or MasterCard (expiry date and signature required).
Fees
Unless otherwise specified, Fees exclude the harmonized sales tax (HST).
